GROUP CAMPING TRIP WAIVER FORM
Thank you for your interest in joining us for Outdoorithm Collective’s Group Camping Trip! To ensure the safety and well-being of all participants, we require each adult participant to complete and sign a waiver form.
If you are attending with minors, you will also need to provide their details in your waiver form.
This waiver and photo release apply to all 2025 Outdoorithm Collective group camping trips, regardless of the number of trips you attend, unless the signer revokes the waiver or release in writing 30 days prior to the trip.

By completing and submitting this form, you hereby acknowledge and agree to the following terms as a condition of participating in the Outdoorithm Collectives’s 2025 Group Camping Trips.
Acknowledgement of Risks: I understand and acknowledge that participating in outdoor camping activities involves certain risks, including, but not limited to, physical injury, illness, death, or loss or damage to personal property. These risks may arise from natural hazards, wildlife, weather conditions, and other participants' actions.
Assumption of Risk: I voluntarily assume all risks, both known and unknown, associated with these activities, even if arising from the negligence of the organizers, and assume full responsibility for my participation.
Compliance with Instructions: I agree to observe and obey all posted rules and warnings, and further agree to follow any oral instructions or directions given by Outdoorithm Collective, its officers or directors, or the staff or volunteers responsible for the event.
Fitness to Participate: I declare that I am physically fit and have not been advised otherwise by a qualified medical professional.
Minor Participants: (If applicable) As the parent/legal guardian of the minors listed in this form, I consent to their participation and acknowledge that the minor is also bound by all the terms of this waiver.
Release and Waiver of Liability: I hereby release, waive, and discharge both Outdoorithm, LLC and Outdoorithm Collective, their officers, directors, agents, and employees from any and all liabilities, claims, demands, actions, and causes of action whatsoever directly or indirectly arising out of or related to any loss, damage, injury, or death that may be sustained by me while participating in such activity.
Indemnification: I agree to indemnify and hold harmless both Outdoorithm, LLC and Outdoorithm Collective from any loss, liability, damage, or costs, including court costs and attorney fees, that may occur due to my participation in said activity.
Photography/Video Release: I understand that during the event, I and/or the minors in my care may be photographed or recorded. I agree to allow my/their photo, video, or film likeness to be used for any legitimate purpose by Outdoorithm Collective.
Acknowledgment of Understanding: I have read this waiver of liability and fully understand its terms. I acknowledge that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
